Urineonderzoek
Heeft u klachten bij het plassen? Zoals u van ons gewend bent kunt u binnen onze openingstijden urine langsbrengen op de praktijk. U kunt een potje urine vanuit huis meenemen, of bij ons op de praktijk een potje ophalen en gebruik maken van het toilet voor patiënten. U kunt hiervoor onderstaand formulier invullen.
Let op! Vermeld altijd aan de assistente wie u bent, of u het formulier online heeft ingevuld of ter plekke wilt invullen en overhandig de urine persoonlijk aan de assistente. Zo weten wij zeker dat wij het juiste onderzoek voor de juiste persoon doen.

Are you experiencing discomfort when urinating? Feel free to come by and leave a urine sample. You can either bring a jar from home or get a jar at the front office and go to the bathroom here. Please make sure to fill out the form below.
Please note the following (!): always tell the front office employee who you are, whether you filled out the form online or if you would like to fill out in person. Make sure you always hand the urine sample to the front office employee. This way, we can ensure we are working on the proper sample for the right person.
